Step 1

To ensure you or your loved one has a safe living environment, we offer a free 100 point home safety inspection that covers all living areas inside your home as well as the areas around the outside of your home. We want to make sure we take immediate action to prevent avoidable injuries.      

Step 2

After completing your home safety inspection and taking the necessary corrective actions to improve the safety of your home, it’s now time for us to get to know you and your family. We’ll meet in your home, to conduct a personal assessment and discovery session. This process will ensure you or your loved one and Our Crew are working together toward your care goals. We will cover a variety of items, such as: your medical conditions, activities of daily living, your medication list, your favorite foods and so much more! An Elder Crew RN will review your personal assessment and create a unique care plan designed to meet your care goals.

Step 3

Now that we’re established your care goals, and created your personal care plan, it’s time for us to get to work! Depending upon your personal needs, we’ll begin our services to your loved one immediately! We will select caregivers for specifically for you or your loved one based off your individual preferences and requirements learned from your personal assessment and our discovery session.  

We are proud to offer the most advanced software in the market to manage your loved ones care and also bridge communication between our caregivers and your loved one. We offer 24 hour, real time information about your loved ones care. Our system offers:

  • Access to The Elder Crew Family Room:

    • Centralized secure access to your personalized care plan, daily tasks, medication lists, calendar events, with the ability to add or view notes.

    • Our clients' loved ones, and the other members of your health care team (Physicians, Assisted Living/Hospice or fiduciaries), can be granted access to your personal secure Elder Crew Family Room site.

  • Elder Crew caregiver visits are accurately recorded by GPS based functionality within our mobile application. You’ll know when our caregiver has arrived and when they leave.
